FUSE PARENT INFORMATION
Please select the campus your registered camper will be traveling with to FUSE Camp to access important information regarding how to prepare for camp, check-in, departure/return times, packing lists, camp expectations, and more.

PRESCRIPTION MEDICAL FORM
If your student will be taking prescription medication while at FUSE, you will need to fill out a Medication Administration Form and turn it in at check-in along with a 5 day supply of their medication in it's original prescription container. Please do not send over the counter medications with your camper. Our medical team will be fully supplied and can dispense those as needed.
CROWER’S RIDGE REGISTRATION
Crowder’s Ridge is the location of FUSE, and they require a registration of their own (including their release and dietary needs). Click the button below to fill out the form for each student attending FUSE.
To complete your Crowder's Registration, you will need to enter a group code.

PORT CITY WAIVER
Whew! You're almost done!
Your final step is to fill out the 2024 Port City Waiver for each student.
*If your student attended the Winter Retreat, you don’t need to fill this out again. Woo hoo!
FUSE 2024 FINAL PAYMENT
Final Payments Due June 3rd.

We know that the cost of camp can be a lot, but we never want finances to keep a student from being a part of FUSE!
